Great Old Ones are Entity come from the First Gate, the place that exist and non-exist outside and beyond all the creation at the same time. They are too strong that their presence alone are enough to collapse the creation, that why every time they appear, it's just their avatar, which is infinite time weaker than their true form. The reason why they usually interact with other being in the lower dimension is unknown but it said that every events that happen always have their mark in it. Some say that they are the master mind behind all of this, some say that they just like to watch these events
